This was most recently documented at 202 S Perkins Road, Elsie, Nebraska 69134 on June 12, 2019. The employer failed to ensure that the lockout procedures used by employees to service equipment were specific for locking out equipment, including, but not limited to, the 86 foot Drag Conveyor.
