29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: a) Sizing Department: Employees were required to wear respirators and allowed to have facial hair that interfered with the seal of the respirator, on or about June 13, 2019. No abatement certification or documentation required.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section: a) Phase 2 Plant: Employees were required to lock out machinery while doing daily servicing and the facility did not have specific procedures for each of the machines, on or about June 19, 2019. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
