29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system.  On or about June 14, 2019, an employee working on a ladder jack scaffold approximately 14 feet above grade was exposed to fall hazards.  Fall protection was not being utilized.    No abatement certification or documentation is required for this item.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On or about June 14, 2019, an employee working on a ladder jack scaffold approximately 14 feet above grade was exposed to fall hazards.  The employee had not received training to recognize and minimize the hazards associated with scaffold use.   Abatement certification is required for this item in accordance with 29 CFR 1903(c).

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about June 14, 2019, an employee working below siding operations taking place on a ladder jack scaffold was exposed to overhead hazards.  The employee was not wearing a hard hat or other protective helmet.    No abatement certification or documentation for this item is required.
