At the establishment, employees operating a Hyster 80 4000 pound forklift, were not trained by the employer in the safe operation of the forklift, exposing employees to the hazard of being struck by the lift.

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths without splice or tap:      a)At the establishment, in the production area, a yellow extension cord supplying power to a Dewalt saw was severely cut, frayed in several places on the cord, exposing employees to the hazard of electrical burns and shock.    b) At the establishment, employees used a black power cord to power a Dewalt angle grinder.  The black cord was spliced to a red extension cord and wrapped with black tape, exposing employees to the hazard of electrical burns and shock.
