29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a) Montefiore Hospital - Roadway Adjacent to 29 Glover Johnson Place, New Rochelle, NY: Employees cutting asphalt concrete on the roadway were required to use N95-type respirators and the employer did not develop a written respiratory protection program that specified that respirator was required during asphalt cutting operations and the specific respirator type to be used. b) Montefiore Hospital - Roadway Adjacent to 29 Glover Johnson Place, New Rochelle, NY: Employees cutting asphalt concrete on the roadway were required to use N95-type respirators and the employer did not implement a written respiratory protection program in that employees did not receive a medical evaluation and fit testing prior to starting work; on or about 6/3/19.
