29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions:    a)  Duckels Construction, Inc. 1360 Indian Trail, Steamboat Springs, CO  80487:  On and before June 17, 2019 the employer did not ensure daily inspections of excavations, the adjacent areas, and protective systems were made by a competent person for evidence of possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions.  Employees dug a trench with vertical walls approximately 10 feet deep to install/replace a main water line at the Copper Mountain Ridge Estates using a CAT track-hoe.  An employee entered the trench on the ramped side, to approximately 6 feet deep, without protective systems.  The employer did not ensure that a competent person inspected the trench prior to entry.  This condition exposed employees to cave-in hazards.
