These fall hazards included: a. 1st story 14 feet and b. 2nd story 24 feet.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about June 13, 2019, Able Roofing, Inc. exposed employees to fall hazards, while accessing and exiting the roof via two extension ladders that did not extend at least 3 feet above the upper landing surface of the roof, on a residential construction site in Bloomington, Illinois. These instances included: a. Extension ladder to 1st story 1 foot above the roof and b. Extension ladder to 2nd story 1 foot above the roof.
