29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:    a)  On or about May 24, 2019 and at times prior, General Air Service and Supply Company, as the creating employer, exposed employees of Mayo Aviation to struck-by, explosion and fire hazards by installing a ball valve rated at 3000 psi and not cleaned for oxygen service in a shop-made oxygen transfilling system that did not contain suitable pressure regulating devices where compressed oxygen was supplied from a Haskell Booster model #17443 (capable of supplying up to 4500 psi).  Additionally, the system contained compressed gas cylinders rated at less than 4500 psi; pressure gauges rated at 4000 psi; and a regulator rated below 4500 psi.  This regulator also provided no control over the compressed oxygen introduced into all portions of the manifold nor did the manifold system contain a pressure relief device to relieve excess pressure.  This is not in accordance with section 3.4.5 and 3.4.7 of Compressed Gas Association Pamphlet P-1-1965.
