On or about June 24, 2019, Production Area: An employee with a full beard regularly used a 3M tight-fitting half-mask respirator fitted with 3M 2019 P100 filters while cutting fiberboard blanket. The beard prevented a tight seal to the employee's face.

On or about June 24, 2019, Shop: The company operated a Hyster Model H50XL2, S/N C177B09559N forklift with standard 42-inch forks. The site manufactured 6-foot fork extensions for use with the vehicle. The data plate did not identify the down-rated capacity of the vehicle when used with the extensions.
