(a) New Cobb County Fire House located at 5781 Mableton Parkway, Mableton, GA 30126: On or about June 11, 2019, and at times prior, the employer exposed employees to struck-by hazards while flying 64-ft., 6-in. trusses onto load bearing walls, in that the signal person used was not qualified and was not evaluated, tested, or certified by a qualified evaluator. No documentation was made available at the jobsite regarding who the signal person was and what signals they were qualified for.
