29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  Fulfillment Area:  An employee was exposed to an 8 to 10 foot fall  when the employee opened the gate of a Work Assist Vehicle and then stepped onto a storage rack with her right foot and then climbed onto a side mid rail of the WAVE with her left foot while trying to reach a bundle of boxes to cut the strapping around the bundle. The employee fell and suffered multiple injuries while trying to return to the operating platform.
