29 CFR1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  On or about June 18, 2019, employees were exposed to a struck by hazard while operating a JLG Sky Power 860SJ Boom Lift without instruction in its safe operation.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e):  On or about June 18, 2019, and at times prior thereto, employees were exposed to a fall hazard while operating a JLG Sky Power 860SJ Boom Lift without fall protection.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about June 18, 2019, and at times prior thereto, employees were exposed to a fall hazard while replacing a commercial roof system without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 18, 2019, and at times prior thereto, employees were exposed to a fall hazard while climbing down a Werner extension ladder off the commercial roof.
