29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): On or about 6/17/2019 in the production and welding areas located at 325 Lower Boundry Street, Macon, GA: Employees operated powered industrial trucks with defects and needed repairs such as but not limited to loose backrest bolts, damage hydraulic lines and pulleys, right front light not working, both rear lights not working, hydraulic cylinder leaking, oil level stick handle broken, corrosive material built-up on the terminal, and battery hold down bracket not secured. The employer failed to take the lifts out of service until restored to safe operating condition.

29 CFR 1910.179(j)(2): The following items were not be inspected for defects at intervals as defined in paragraph (j)(1)(ii) of this section or as specifically indicated, including observation during operation for any defects which might appear between regular inspections: On or about 6/17/19 in the production area at 325 Lower Boundary Street, Macon, GA: a) Employees using the 30 ton Demag Crane were exposed to struck-by hazards of falling materials when the employer did conduct frequent inspections on the crane and crane hook. The crane hook had was not marked for the rated capacity. b) Employees using the 15-ton Demag Crane were exposed to struck-by hazards of falling materials when the employer did not conduct frequently inspections the crane and crane hook for deficiencies. The crane hook safety latch missing. c) Employees using the 2-ton Budget Crane were exposed to struck-by hazards of falling materials when the employer did not conduct frequently inspections the crane and crane hook for deficiencies. The crane trolley hoist with hook was not properly resting on the bridge rails.
