The most recent occurrence was evident during the inspection conducted on June 18, 2019 at a worksite located at 20926 Larimore Ave, Elkhorn, NE 68022. The employer has a 32 foot long trench that varies from 8 to 12.3 feet in depth at the jobsite. There are spoils piles located on both sides of the trench. The spoils pile on the east side of the trench was located right on the edge.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system: The employer is failing to protect employees from crushing hazards while working in a trench. The most recent occurrence was evident during the inspection conducted on June 18, 2019 at a worksite located at 20926 Larimore Ave, Elkhorn, NE 68022. The employer allowed his employees to enter a trench, which is about 32 feet long, 12.5 feet wide, and 8 to 12.3 feet deep, even though it was not secured by a protective system.
