29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. (a) Employees conducting roofing activities on a roof with a 12:12 pitch were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 20 feet.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. (a) The employer did not ensure that employees exposed to potential fall hazards while working were properly trained on the recognition and minimization of fall hazards at the worksite.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support). (a) Access ladder for the roof was not angled at a proper ratio relative to its height. Where the roof height was approximately 20 feet, the ladder should have been 5 feet out from the side of the building, when it was approximately 4 feet out.
