(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:      a.  Located at 1191 Fields Avenue, Columbus, Ohio:  On or about June 19, 2019, the employer failed to ensure that employees were protected from falls, while employees performed lubrication job orders for scheduled maintenance, on top of the waste and recycling trucks, with unprotected side and edges.  The employer exposed the employees to a fall distance of 10 feet 7 inches to the ground level, without any means of fall protection.    Rumpke Consolidated Co., Inc., was previously cited for a violation of this Occupational Safety and Health standard, which was contained in OSHA inspection number 1290267, citation number 1, item number 2a and was affirmed as a final order on March 20, 2018, with respect to a workplace located at Greenville Transfer Station Facility, Greenville, OH 45331.

29 CFR 1910.30(a)(1): (a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart.  Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.    a.  Located at 1191 Fields Avenue, Columbus, Ohio:  On or about June 19, 2019, the employer failed to provide training for each employee that are exposed to fall hazards.  Employees were not trained in the nature of the fall hazards in the work area and how to recognize them. Employees were exposed to a fall distance of 10 feet 7 inches to the ground level, without any means of fall protection, when conducting lubrication orders on top of recycling and waste trucks.
