On or about June 19, 2019, at the above addressed jobsite, an employee conducting roofing operations roof, approximately seventeen (17) feet above the ground, had a lifeline secured to roof anchor plate with one (1) nail total, thereby exposing the employee to fall hazards.

R & S Roofing Company was previously cited for a violation Occupational Safety and Health Standard 1926.501(b)(13) which was contained in OSHA inspection number 1301899, citation number 1, item number 1 and was affirmed as a final order on May 3, 2018, with respect to a workplace located at Lot 84, Whisper Creek Way, Mokena, IL.    R & S Roofing Company was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1926.501(b)(10) which was contained in OSHA inspection number 1236811, citation number 1, item number 1a and was affirmed as a final order on August 23, 2017, with respect to a workplace located at 7301 Willow Springs Road, Countryside, IL.
