OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to fire hazards resulting from the ignition of flammable vapors: Jobsite: The employer did not ensure through visual observation and/or specific instruction that the task, such as (but not limited to) purging a propane gas pipeline that was providing fuel to the air handler of the HVAC Unit located in the unfinished attic was done far away from the electrical ignition source. On or about 06/19/2019, propane fume was ignited while two (2) employees were performing the above task and both employees were transported to the hospital. One employee sustained with first and second degree burns to one of arms, face and legs, and the other employee sustained with the second and third degree burns to the face and both arms.
