29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:     a) Jobsite: On June 05, 2019 and at times prior thereto employees performed framing activities from a scaffold platform elevated by a rough-terrain forklift. The scaffold platform was not secured to the rough-terrain forklift, exposing employees to a 12 to 17 foot fall hazard from the platform to the ground below. 29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) Jobsite: On June 05, 2019 and at times prior thereto the employer did not protect workers from 12-17 foot fall hazards when working from a scaffold platform. Employees engaged in framing activities were permitted to work from a scaffold platform without fall protection to gain access to elevated sections of the residential structure.
