29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: 1) Pallet Changer station: On or about June 19, 2019, the employer exposed employees to caught-in hazards, in that lockout/tagout procedures were not developed and implemented on the Cascade Pallet Changer serial 850208-Ti and model 40A for clearing jams.
