29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  On or about June 18, 2019, employees were conducting roofing work on the commercial building up to approximately 15 feet high without fall protection, exposing employees to a fall hazard.  Texas Fifth Wall Roofing Systems was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1086078, citation number 1, item number 3 and was affirmed as a final order on September 24, 2015, with respect to a workplace located at 2627 SE Military Dr. San Antonio, TX 78249.  Texas Fifth Wall Roofing Systems was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1145592, citation number 2, item number 1 and was affirmed as a final order on November 15, 2016, with respect to a workplace located at 11752 Potranco Rd. San Antonio, TX 78253  Texas Fifth Wall Roofing Systems was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1153185, citation number 2, item number 1 and was affirmed as a final order on December 29, 2016, with respect to a workplace located at 1710 Corner Way Blvd., San Antonio, TX 78219  Texas Fifth Wall Roofing Systems was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1208985, citation number 1, item number 1 and was affirmed as a final order on May 17, 2017, with respect to a workplace located at 1 Spur Lane, San Antonio, TX 78420
