29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees. On or about 19 June 2019, and at time prior thereto, a worker was inside a trench and excavation that was at least 4 feet deep without a ladder for egress, exposing the worker to a fall hazard of approximately 4 feet or more.

29 CFR 1926.651(j)(2): Employees were not protected from excavated or other materials or equipment that could pose a hazard by falling or rolling into excavations, or by a combination of both if necessary. On or about 19 June 2019, and at times prior thereto, a worker was inside a trench installing electrical lines in which the spoil pile was not at least 2 feet away from the edge, exposing the worker to being struck by falling excavated materials.
