29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition:  (a)  DUST COLLECTION TRAILER AND PLATFORM - The employer did not ensure that the dust collection trailer and the elevated platform that the dust collection system was installed on were free from fugitive wood dust accumulation, on or about June 20, 2019.  In the alternative  29 CFR 1910.141(a)(3)(i): All places of employment were not kept clean to the extent that the nature of the work allows.  (a)  DUST COLLECTION TRAILER AND PLATFORM - The employer did not ensure that the dust collection trailer and the elevated platform that the dust collection system was installed on were free from fugitive wood dust accumulation, on or about June 20, 2019.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment:    (a)  DUST COLLECTION ROOM - Employees operated the light switches for the shop area, and the electrical panel that housed the light switches contained two unused openings that were not covered which exposed employees to the energized conductors within, on or about June 20, 2019.    No abatement certification or documentation required.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  PRODUCTION AREA - Employees used filtering facepiece respirators while performing tasks including wood cutting and sanding, and the employer did not provide the employees with the information contained in Appendix D to 29 CFR 1910.134, on or about August 8, 2019.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
