29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use.        On the east side of the construction site, employees installing decking material worked from a work platform, elevated by a JLG forklift, that was not an approved work platform by the manufacturer for capacity, construction, use and fall protection exposing the employees to fall hazard of more than 15 feet.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:      On the east side of the construction site, employees installing decking material on the 8:12 pitched roof of a new commercial building without fall protection, exposed the employees to an unprotected fall hazard of approximately 15 feet to the ground below.

29 CFR 1926.602(c)(1)(vi): 29 CFR 1926.602(c)(1)(vi):Section 603E Industrial truck(s)  American National Standards Institute, B56.1-1969, Safety Standards for Powered Industrial Trucks, as adopted by 29 CFR 1926.602(c)(1)(vi): When a truck was left unattended, load engaging means were not fully lowered, controls were not neutralized, power was not shut off, brakes were not set, and the key or connector plug was not removed.      On the east side of the construction site, the employer allowed the JLG 6042 forklift cab and controls to be unattended while employees installed decking material from the elevated forks, exposing the employees to struck-by and fall hazards.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On the east side of the construction site, employees installing decking material on the 8:12 pitched roof of a new commercial building were not provided training on fall hazards and fall protection, exposing the employees to an unprotected fall hazard of approximately 15 feet to the ground below.
