29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On 06/18/19 at worksite 237 W 4th St, KC MO -- Employees were exposed to fall hazards while performing work approximately 30 feet above the ground without any fall protection on. Employees were unloading drywall that was lifted up to them via a all-terrain fork truck. The employees removed the guardrails and were not tied off.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  On 06/18/19, worksite at 237 W 4th St, KC MO -- Employees were exposed to a fall of approximately 30 feet to the ground. One employee was wearing a harness with rope while unloading drywall from a all-terrain fork truck. The anchorage point was not designed to support 5,000 pounds. The anchor point was not appropriate either, as it was a lanyard wrapped around a wall stud to a rope. The lanyard was not designed to be used as an anchor.
