29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting as location requires. Employees engaged in oil and gas operations in the mud mixing area were exposed to caught-in hazards in that the horizontal rotating shaft for the Twin Disc power transmission equipment located approximately 20 inches from the work platform was not adequately guarded. Exposed sections of the rotating shaft measured approximately one inch, and one and one half inches in width.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o). Employees engaged in drilling operations, accessing the work platform adjacent to the Detroit Diesel Series 60 generator were exposed to caught in and amputation hazards in that the lower pulleys located approximately 22 inches from the work platform were not fully guarded.
