29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph: a) Westerly end of roof , on or about 6-7-19: Two employees handled an energized metal service entrance mast assembly, in an effort to tie it back and support it during construction, exposing the employees to a severe electrical shock and/or electrocution.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Westerly end of the building, Unit #13, on or about 5-8-19: Two employees were working on a roof surface, 5:12 pitch, without fall protection, exposing the employees to a fall hazard of 13 feet or more to the pavement below.
