29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) 1144-46 Frankford Ave., Philadelphia, PA 19125 - The employer did not ensure that an employee who was conducting demolition operations on the second floor of a building with unprotected sides approximately 11 feet above the lower level was protected  from falling by guardrail systems, safety net systems, or personal fall arrest systems, thus exposing the employee to fall from height hazards.  This condition was noted on June 19, 2019.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a) 1144-46 Frankford Ave., Philadelphia, PA 19125 - The employer allowed employees, who were conducting demolition operations, to enter and exit a basement approximately 5 feet below the upper landing using a self-supporting ladder in the closed position, thus exposing employees to fall hazards. This condition was noted on June 19, 2019.
