On or about June 20, 2019, on south corner of building, an employee was exposed to a fall hazard while attaching metal panels on an exterior wall of a junior high school building from a Genie aerial lift basket without a fall protection system.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. On or about June 20, 2019 on the southside of the building, an employee was exposed to fall hazards of more than 6 feet while fixing leaks in the roof without a fall protection system.
