(a) Site: In the back room assembly and receiving area, 510 North Main Street, Leominster, MA 01453 on 6/18/2019, the employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces in that employees were observed climbing from the loading dock into a trailer and back without the use of a ladder or steps. The distance the employees climbed was measured to be approximately 40 inches.
