On or about June 18, 2019, during the installation of roofing material at the job site located at 21 N. Hametown Rd, Akron, Ohio 44333; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.
