Employees were exposed to 18 foot falls, eye injuries when operating equipment, and falling object hazards when performing roofing work.

Blue Chip Construction LLC site located at 833 N. Main Street, Lima, Ohio:  On or about June 21, 2019, the employer did not ensure employees were protected from fall hazards of 9 feet 2 inches when cutting roof sheeting while standing on top of the first story roof.
