Personal fall arrest systems when stopping a fall must: 29 CFR 1926.502(d)(16)(iii): be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level; On and about June 21, 2019, employees sheathing the roof of a pole barn located at 3644 Nekimi Ave. Oshkosh, WI were exposed to falling greater than 6 feet between the trusses and purlins and from the eave edge and gable end when their lifelines allowed lateral travel farther than the distance to the edges.

29 CFR 1926.503(a)(2)(viii): The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the following areas: The standards contained in subpart M. On and about June 21, 2019, employees sheathing the roof of a pole barn located at 3644 Nekimi Ave. Oshkosh, WI were exposed to falls when the employer did not ensure fall protection training included information contained in 1926 Subpart M, including but not limited to swing fall, leading edge procedures, and harness adjustment.
