29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:  On or about June 20, 2019, employees performing roofing activities were exposed to fall hazards of approximately 21 feet and the employer did not ensure fall protection was used around open skylights.  An employee fell through an open skylight and struck the ground approximately 21 feet below.

WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.503(b)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1352961, CITATION NUMBER 1, ITEM NUMBER 1c AND WAS AFFIRMED AS A FINAL ORDER ON DECEMBER 3, 2018, WITH RESPECT TO A WORKPLACE LOCATED AT 1275 AVENUE OF THE CITIES, UNITED TOWNSHIP HIGH SCHOOL, EAST MOLINE, IL 61244.
