29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At this worksite, employees engaged in roofing duties used portable ladders to access/egress the roof. The employees were exposed to fall hazards when the ladders did not extend three feet over the roof landings in the following instances: a. section of extension ladder used to access/egress from the initial roof and b. ladder used to access/egress the second roof.
