On or about June 21, 2019 the employer did not assure the 20 foot portable Werner brand aluminum articulating ladder was used at an angle such than the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length. The foot of the ladder was measured at 8 feet from the wall the wall height was determined to be 11 feet- 3 inches from the ground.

Olveras Construction Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501 (B)(13) which was contained in OSHA inspection number 1309362, citation number 1, item number 1 and was affirmed as a final order on June 11, 2018, with respect to a workplace located at 129 Cerise Court Lot #279 Ormond Beach, FL 32174
