29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): On or about 06/24/2019, at 400 S. Dixie Highway in the city of Hollywood, Florida, employees who were operating a powered industrial truck (PIT) and had received PIT training had not been certified by the employer as operators, exposing employees of the warehouse to the potential of struck-by hazards.
