29 CFR 1926.451(a)(6): Scaffolds were not loaded in accordance with the design by a qualified person:  (a) On or about June 19, 2019, and at times prior, the employer Ironwood Group LLC, did not ensure that employees operating and working from a Skyjack SJIII Series aerial platform model 3219 were loading it in accordance with its design in that employees were hoisting down steel pipes approximately 20 feet in length and weighing approximately 260 pounds. Skyjack SJIII Series Operating Manual states "Do Not use the aerial platform as a crane. It is prohibited.  Do Not exceed the rated capacity of the aerial platform. Do make sure the load is evenly distributed on the platform."  The steel pipe swung loose while being hoisted and hit the side of the aerial platform, causing it to tip and eject the employees. The employees suffered fractures and contusions.    Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold(s) trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold(s) being used and to understand the procedures to control or minimize those hazards:  (a) On or about June 19, 2019, and at times prior, the employer Ironwood Group LLC did not ensure that employees operating and working from a Skyjack SJIII Series aerial platform model 3219 were trained to recognize hazards associated to the aerial platform and how to control or minimize those hazards. The employees were hoisting down steel pipes approximately 20 feet in length and weighing approximately 260 pounds.   The steel pipe swung loose while being hoisted and hit the side of the aerial platform, causing it to tip and eject the employees. The employees suffered fractures and contusions. 29 CFR 1926.454(b): The employer did not have each employee who was involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question:  (a) On or about June 19, 2019, and at times prior, the employer Ironwood Group LLC did not ensure that employees operating and working from a Skyjack SJIII Series aerial platform model 3219 were trained by a competent person to recognize any hazards while hoisting down steel pipes approximately 20 feet in length and weighing approximately 260 pounds.   The steel pipe swung loose while being hoisted and hit the side of the aerial platform, causing it to tip and eject the employees. The employees suffered fractures and contusions.
