29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At the jobsite, accessing the porch roof, on or about 6/24/19: Employees were using four separate ladders to access a 2:12 pitch porch roof with ladders that did not extend at least 3 feet above the upper landing, which was 14 feet from ground. b) At the jobsite, accessing the main roof, on or about 6/24/19: Employees were installing using two separate ladders to access a 9:12 pitch roof with ladders that did not extend at least 3 feet above the upper landing which was 10 feet from the lower level (porch roof).
