29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6): a) On or about 6/21/2019, Employee suffered a finger tip amputation while performing servicing and maintenance on Machine T62 that was not de-energized and came in contact with an unguarded knife roll during the re-webbing of Plow #1 & #2 .

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.: a) On or about 6/26/2019, employees cleaning at the end of their shift on R65 used an air wand without the pressure reducing tip in good shape. Dead head pressure on the tips would have been in excess of 30 p.s.i. as the HUSKY PRO air compressor in the shipping department had a gauge on it with a reading of 90 p.s.i. b) On or about 6/26/2019, employees cleaning at the end of their shift on Machine T62 folder area used an air wand without the pressure reducing tip in good shape. Dead head pressure on the tips would have been in excess of 30 p.s.i. as the HUSKY PRO air compressor in the shipping department had a gauge on it with a reading of 90 p.s.i. c) On or about 6/26/2019, employees cleaning at the end of their shift on Machine T62 packing station area used an air wand without the pressure reducing tip in good shape.
