One employee was not attached to the fall arrest system (b) Employees installing roofing for a new residential unit were exposed to fall hazards over 8 feet to 21 feet to the ground. Two employees did not adjust rope systems to prevent contact with ground in the case of a fall. NOTE: Certification of corrective action is required for this item.
