29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      a) Third Villa West: On or about June 20, 2019, two workers were exposed to falling 19.5 feet from a residential multifamily unit while performing roofing work without adequate fall protection.      Abatement Note: The employer corrected the violations while CSHO was on site.      NOTE: Beacon Restoration, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1400160, citation number 01, item number 001, and was affirmed as a final order on June 12, 2019, with respect to a workplace located at Garages East of Button Rock Dr., Longmont, CO 80504.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) Third Villa West: On or about June 20, 2019, two workers demonstrated lack of effective training while working on a roof 19.5 feet above the next lower level by disconnecting from rope lanyards and connecting their lifelines in an incorrect manner by attaching the wrong end to their rope grab devices.    Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).
