29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazard: Worksite: The employer did not ensure through selecting and providing appropriate personal protective equipment (PPE) to protect employees from the hazards associated with operating awn mowers. On or about June 18, 2019, the employee wearing sneakers while operating a push lawn mower slipped, and the right foot went under the mower coming in contact with the rotating blade resulting in an amputation of four toes.
