29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. (a) Workers supplying roofing materials were not anchored with lanyards to the Genie S60 aerial lift use to access the approximate 30 feet high peak of a 13:12 steep pitched roof. NOTE: Certification of corrective action AND documentation as evidence of abatement are both required for this item.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: (a) The D-ring attachment point on the harness of a worker was in the lower portion of his back. The worker was at the peak of a steep sloped roof with a peak height of approximately 30 feet. NOTE: Certification of corrective action is required for this item.
