29 CFR 1910.147(e)(2)(i): Before lockout or tagout devices are removed and energy is restored to the machine or equipment, procedures shall be followed and actions taken by the authorized employee(s) to ensure the following: "Employees" The work area shall be checked to ensure that all employees have been safely positioned or removed. a) On 6/21/2019 an employee conducting testing and positioning on a SMT Tube Bender (ID #21188) was not safely positioned to ensure that while checking measurements on the bender during setup that the employee was not standing with foot on pedal and able to activate the bender.

29 CFR 1910.147(f)(1)(ii): In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the following sequence of actions shall be followed: Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section; a) On 6/21/2019 during setup of the SMT Tube Bender (ID #21188) when an employee has to check the measurements for the bender during testing and positioning of the equipment, the employer did not ensure that the employee was safely positioned away from the foot pedal to prevent activating the equipment.

29 CFR 1910.212(a)(1): "Machine guarding" Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) On or about 6/21/2019 an employee was operating a SMT Tube Bender (ID 21188) and was exposed to a caught in hazard while conducting setup operations due to the lack of safe guards to protect the operator. The safety mat allowed the employee to be exposed to a caught in hazard due to the mat not working properly. The employees could activate the foot pedal while standing on the safety mat.
