29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a)  Workers engaged in sheathing the second level of a new two story residential unit were exposed to fall hazards from 20 to over 25 feet to the ground.    (b)  A worker engaged in handing sheathing from the first to second level of a new two story residential unit was exposed to a fall hazard of 10 feet from the ground to eave.    NOTE: Certification of corrective action AND documentation as evidence of correction are both required for this item.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a)  Workers used a closed 8 foot A-frame step ladder to access the upper roof from the lower roofline.  The ladder only extended approximately 1 foot above the upper roof landing.    NOTE:  Certification of corrective action is required for this item.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  (a)  Workers used a closed 8 foot A-frame step ladder to access the upper roof from the lower roofline.  A-frame ladders are designed to be used on flat surfaces with in the fully open position with spreaders completely extended.  NOTE:  Certification of corrective action is required for this item.
