Triple Diamond Plastics, Inc. located in Liberty Center, OH:  On or about June 26, 2019, the floor around press #6 was covered in water creating a slip hazard for employees working in the area.    b. Triple Diamond Plastics, Inc. located in Liberty Center, OH:  On or about June 26, 2019, the employer did not ensure maintenance employees were provided with an arc rated hard hat to protect against potential arc flash hazards when exposed to 480 volt energized conductors inside the main electrical cabinet while shutting off the hydraulic pump on the #3 plastic injection molding machine.    d. Triple Diamond Plastics, Inc. located in Liberty Center, Ohio:  On or about June 26, 2019, the employer did not ensure air monitoring had been conducted to determine employee exposures to formaldehyde while an employee was operating plastic injection molding machine #2.  An employee was exposed to formaldehyde at 0.024 ppm when operating the equipment for a sampling period of 236 minutes.
