29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: (a) On or about 06/20/2019 - 736 Wallace Wade Drive, Tuscaloosa, AL, East Building, North Side, 2nd Floor, an employee utilizing a Guardian retractable lanyard that was stretched out approximately 10 feet from the anchor point, was exposed to a free fall distance of approximately 8 feet resulting in the employee coming in contact with a support column and capped rebar located at the ground level.
