On or before June 26, 2019, where employees used an extension cord to provide electrical power to an air compressor, the ground prong was missing from the male end of the extension cord, thereby exposing employees to a 123 volt electrical contact hazard.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.    a. On or before June 26, 2019, where employees used an air compressor to provide compressed air for pneumatic nail guns, the strain relief was pulled away from the outer insulation jacket of the cord exposing the interior conductors, thereby exposing employees to a 123 volt electrical contact hazard.

On or before June 26, 2019, the employer failed to initiate and maintain a safety and health program such as but not limited to an electrical safety program, thereby exposing employees to an electrical contact hazard.    b. On or before June 26, 2019, the employer failed to initiate and maintain a safety and health program such as but not limited to a fall protection program, thereby exposing employees to a fall hazard.    Mena LLC was previously cited for a Repeat violation of this occupational safety and health standard, which was contained in OSHA inspection number 1206499, citation number 1, item number 1 and was affirmed as a final order on April 10, 2017, with respect to a workplace located at Lot 1756 Sunbury Ridge, Sunbury, OH 43074.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.  Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems.    a. On or before June 26, 2019, where employees were framing the second story of a single family house, the employees were not protected from falls, thereby being exposed to a 13 foot fall hazard.    b. On or before June 26, 2019, where employees were framing the second story of a single family house, the stairwell was not guarded, thereby exposing the employees to a 10 foot 4 inch fall hazard.    Mena LLC was previously cited for a Repeat violation of this occupational safety and health standard, which was contained in OSHA inspection number 1206499, citation number 2, item number 1 and was affirmed as a final order on April 10, 2017, with respect to a workplace located at Lot 1756 Sunbury Ridge, Sunbury, OH 43074.    Mena LLC  was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 998319, citation number 1, item number 1 and was affirmed as a final order on November 6, 2014, with respect to a workplace located at 1567 Scarlet Avenue, Lewis Center, OH 43035.
