29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:  Employees were exposed to struck-by and fall hazards.  At the jobsite located between 8705 Mackey St. Overland Park, KS:  Employees engaged in roofing activities were exposed to struck-by and fall hazards while working without hard hats while under other employees and working from heights greater than 6 feet above a lower level.  Employees were exposed in that the employer had not developed and maintained a written comprehensive safety and health program which addressed these and other hazards.

At the jobsite located at 8705 Mackey St,, Overland Park, KS:  Employees engaged in residential roofing activities from a 14:12 pitch roof were exposed to fall hazards of approximately 9 feet 3 inches from the eave of the residence to the ground below in that fall protection was not provided.

At the jobsite located at 8705 Mackey St. Overland Park, KS:  Employees engaged in residential roofing activities from a 14:12 pitch roof were exposed to fall hazards of approximately 7 feet 9 inches from the eave of the residence to the porch below in that the ladder used to access the roof was not extended at least 3 feet above the eave and was not secured to the structure.
